Who Is Eligible

VNA Foundation funds grants to nonprofit organizations offering home – and community-based care to the medically underserved in the following areas:

Home health care services
Community and school-based services
Primary care and chronic disease management
Health promotion

Our grant recipients are agencies in the Illinois Counties of Cook, DuPage, Kane, Lake, McHenry and Will.

All grants are made to qualified nonprofit agencies exempt from federal income tax under section 501(c)(3), and designated as public charities under section 509(a)(1) or 509(a)(2), of the Internal Revenue Code.
