A six-month and twelve-month report are required for all VNA Foundation grants unless otherwise requested in your grant award letter or by your Program Officer. Please submit your report through the online Grants Management System by logging into your account and selecting the appropriate report/update form.
Note: Please refer to your grant award letter for report deadlines and your grant reference number.
If you’re a grantee interested in renewal funding or currently have a multiyear award, please refer to your VNA Foundation award letter for reporting deadlines, and then contact Program Officer Rachel Baird to discuss acceptability, timing of a renewal application, and renewal procedures.
Yes, VNA Foundation considers renewal funding, but renewal opportunities must be discussed with VNA Foundation staff. We ask that you contact Program Officer Rachel Baird between the 6th — 8th month of your current grant for more information.
Multiyear grant recipients are also required to contact their Program Officers to determine end-of-year report criteria.
Yes, six and twelve month reports (also called follow-ups) are required of all grantees, unless otherwise requested by the Foundation. Please refer to your grant award letter for due dates. Additionally, you will receive an automatic reminder and link to report templates (follow-up templates) from our online Grants Management System.
No, our renewal proposals request information on your grant that is ending and for which you are seeking renewal funding, so you don’t need to complete another twelve-month report on that same grant.
Contact your Program Officer to discuss any problems. Dialogue is always encouraged.
Please contact your Program Officer to discuss solutions.
VNA Foundation would like to celebrate and share the accomplishments of our grantee partners. If you have a video, press release, new initiative, award or something else you would like to highlight, please let us know!
