VNA Foundation awards grants to support nonprofit home- and community-based healthcare and health services for medically underserved populations. VNA grants generally fit within one or more of the categories listed to the right, although we will consider applications that fall outside these categories while still meeting our guidelines.
The majority of our grants are made to Chicago-based entities, but we also provide funding throughout Cook County and the surrounding collar counties (Lake, McHenry, DuPage, Kane and Will).
We will consider funding program, operating, and capital grants, including salaries that support core staff. In deference to our roots as the Visiting Nurse Association of Chicago, we often have an interest in programs in which care is provided by nurses.
Offered via home- or community-based settings including mobile and outpatient services (e.g. free clinics, Federally Qualified Health Centers, school-based health centers, home health nursing care, etc.).

Often combined with primary care provision, or as part of violence prevention programs, home visiting services or reproductive health efforts.

Often related to domestic violence and sexual assault services, and occasionally other interpersonal violence prevention or early intervention services.
Offered via home- or community-based settings including outpatient services (e.g. free clinics, school-based health centers, home care doula and nursing services, etc.).
