Dental HouseCalls brings dental care directly to Chicagoans with disabilities or who are elderly and cannot travel to dental offices for necessary treatment. HouseCalls patients live in nursing homes or residential centers, attend day programs for people with disabilities or are homebound.

Diabetes Prevention and Treatment

The AIHSC Diabetes Prevention and Treatment Project provides comprehensive diabetes prevention and treatment, as well as targeted health promotion services for medically underserved men, women and children in low-income communities. Our goal is to improve the health and well-being of the AIHSC target population with a focus on helping American Indians and Alaskan Natives. During the current reporting period, a total of 561 have been served through the project.

Facing Forward to End Homelessness

Housing First Nursing Services

Facing Forward’s program provides permanent, supportive housing to homeless women and families and integrates a nursing case management model to address their healthcare needs. The goal is self-sufficiency and we work toward that by assisting and teaching women to navigate the healthcare system so they can be prepared upon exiting the program.
