The program offers free health care, referrals, and human connection through the Health Outreach Bus to homeless and vulnerable individuals in six Chicago neighborhoods.

The Mobile Dental Program (Van)

The Mobile Dental Van provides direct care and preventive education to reduce inequities in oral health for underserved individuals across Chicago. Program staff collaborate with 19 organizations including schools, FQHCs, older adult residences and agencies serving homeless or disabled individuals.

Building Capacity for Chronic Disease Care

The Extension for Community Outcomes (ECHO) model expands capacity for complex care of common chronic diseases in underserved communities. ECHO-Chicago applies this model of capacity building by using telehealth technologies to bring together University of Chicago specialists and community based primary care providers (PCPs) for case-based, iterative, curriculum-driven up-training of PCPs. ECHO-Chicago works with our community partners to determine the disease areas where there is the greatest need for advanced training. Our current offerings include six disease areas: resistant hypertension, pediatric ADHD, childhood obesity and comorbidities, women’s health, child and youth epilepsy, and hepatitis C.

Michael Reese Research and Education Foundation

Michael Reese Breast & Cervical Cancer Program

The Michael Reese Research and Education Foundation requests support for its Breast and Cervical Cancer Program (MR-IBCCP), at MetroSouth Medical Center (Metro-South), where uninsured women seeking early detection services for breast and cervical cancer receive medical case management and treatment. Funding will offset the salary expense of the program’s nurse case manager, a position integral to MR-IBCCP’s entire operation.

Sexual Assault Counseling

The Sexual Assault Counseling Program helps sexually abused children and adults to work through the trauma of rape and rebuild their lives after sexual violence. We provide healthy coping strategies for establishing the foundation of their healing and reducing the risk of re-victimization.
