Over the last year. the SCH Midwifery Group implemented the Centering model of prenatal care across the practice. Centering is a cost-effective, patient-centered model that combines care, education, and community building in a group setting. Through the project, SCH provide this innovative model of care to 53 women and their partners.
Swedish Covenant Hospital is requesting renewal funding for its two-tiered Chronic Disease Care Transitions program. Funding will support the delivery of home visits, disease management education, home telemonitoring, intervention, and care coordination to 60 underserved patients with chronic diseases following hospital discharge. It will also support the delivery of enhanced home visiting services to a subgroup of these CD patients suffering from malnutrition or uncontrolled diabetes.
